The principal symptoms should be well-characterized, with descriptions of
(1) location, (2) quality, (3) quality or severity, (4) timing, including onset, duration, and frequency, (5) the setting in which they occur, (6) factors that have aggravated or relieved the symptoms, and (7) associated manifestations.

1. Where is it? Does it radiate?
2. What is it like?
3. How bad is it? (For pain, ask for a rating on a scale of 1 to 10.)
4. When did (does) it start? How long did (does) it last? How often did (does) it come?
5. Include environmental factors, personal activities, emotional reactions, or other circumstances that may have contributed to the illness.
6. Does anything make it better or worst?
7. Have you noticed anything else that accompanies it?
